Take advantage of the extras you get as a Humana Healthy Horizons® in Kentucky enrollee. Below, you’ll find information about each available benefit.

Criminal Expungement Services

For enrollees aged 18 and older, reimbursement of up to $340 for criminal record expungement, as allowed per KYCourts.gov per lifetime

To request the criminal expungement services benefit:

  • Call Enrollee Services at 800-444- 9137 (TTY: 711)

After requesting the benefit:

  • Humana will send enrollee a form to request reimbursement of the expungement.

GED Testing
GED test preparation assistance for enrollees aged 18 and older, including a bilingual advisor, access to guidance and study materials, and unlimited use of practice tests. Test preparation assistance is provided virtually to allow maximum flexibility for enrollees. Also includes test pass guarantee to provide enrollees multiple attempts at passing the test.

Healthy Horizons in Kentucky pays the course fee of $125 per section, with 4 total sections, so enrollee can access the GEDWorks preparation materials, resources, practice tests, and tests.

Self-Monitoring Devices – Weight Scale

Enrollees 21 and older under care management may receive 1 weight scale every 3 years. Care Manager approval required.

If you are engaged in one of the applicable care management programs, please speak to your care manager for assistance

Smartphone App for Diabetes Management
For enrollees 18 and older with type 2 diabetes who are not already receiving care management services, unlimited access to an innovative digital therapeutics’ smartphone application for diabetes management

Humana Healthy Horizons in Kentucky will notify eligible enrollees about the program.

To participate, you must:

  • Download the Vida Health app (available in English and Spanish) at no cost from iTunes/Apple Shop or Google Play on a mobile device.
  • Take a health and wellness assessment.
  • Pick a health coach to work with.
  • Begin coaching sessions.

Smartphone Services

Smartphones can provide easy access to health related information and enable members to stay connected to their care team and health plan. Humana members that qualify for the Federal Lifeline program are eligible to receive a free smartphone with monthly talk minutes, text and data.

Help accessing groceries and essentials

We all know the quality of food we eat affects our health. For many people, an unhealthy diet is a common reality.

According to Feeding America, more than 37 million people in the United States (or 1 in 9 Americans) are unable to access healthy food. Accessing healthy food, especially during the ongoing COVID-19 pandemic, has been difficult for many people. There are programs that can help.
