  • Plus network provider: $0 copay
  • In-network provider: $10 copay or $0 copay when visiting a PLUS provider
  • Out-of-network provider: $30 allowance

  • One exam every 12 months from the last date of service
Eyeglass frames
  • PLUS network provider: $250 allowance
