Florida Medicaid plans have a lot in common:

But that doesn’t mean that all Florida Medicaid plans are created equal. Some, like Humana Healthy Horizons in Florida, go above and beyond. We provide extra services and perks that make it easier and more rewarding to take care of your health.

How to choose Humana Healthy Horizons in Florida

If you want to switch from your current Medicaid plan and enroll in Humana Healthy Horizons in Florida, you can:

  • Text “ENROLL” to FLSMMC (357662)
    • Message and data rates may apply.                                                                                  
  • Go to FlMedicaidManagedCare.com
    • Create a Member Portal Account or chat with a virtual enrollment assistant.
  • Call the Helpline to speak with a Choice Counselor at: 877-711-3662

To pick a new plan, you will need:

  • Each member’s birth year
  • Each member’s Medicaid ID number or Gold Card number
  • The account PIN number

Before you can enroll in a Medicaid plan, you must apply for Medicaid benefits. To apply:

To apply, you will need information about yourself and the people in your home, including:

  • Full name
  • Social security number
  • Date of birth
  • Proof of citizenship
  • Income
  • Expenses (housing, childcare, etc.)
  • Assets (bank accounts, property, etc.)

Mandatory enrollment

Once the state determines that you qualify for Medicaid you must enroll in a Medicaid plan. This is called mandatory enrollment. You can choose the plan you want, or the state automatically will enroll you into a plan.

When you first enroll in a Medicaid Managed Care Plan, you have 90 days to try the plan. During this 90-day period, you can switch for any reason to a new plan. After this 90-day period ends, you cannot change health plans until the next open enrollment period, unless you have an event that allows a change.

Open enrollment

The yearly Open Enrollment Period gives mandatory enrollees another chance to change plans for any reason.

If you have Humana Healthy Horizons in Florida and want to stay with us, you do not have to do anything.

If you want to switch to Humana Healthy Horizons in Florida ,you can use one of the three options for switching health plans. If you change plans, you will be locked into that plan for 12 months.

Disenrollment for Cause

If you are a mandatory enrollee and you want to change plans after you are locked in, you must have a state-approved for cause reason. For a list of state-approved for cause reasons, please refer to your Member Handbook or call Medicaid Managed Care/Choice Counseling at 1-877-711-3662 (TTY: 1-866-467-4970), Monday – Thursday, 8 a.m. – 8 p.m., and Friday, 8 a.m. – 7 p.m.

Reinstatement

If you lose your Medicaid benefit, you cannot be enrolled in Humana Healthy Horizons in Florida. If you get your Medicaid benefit back within 180 days (six months) of losing your benefit, and you were enrolled in Humana Healthy Horizons in Florida when you lost your benefit, you will be re-enrolled in our plan. We will let you know in writing if you are re-enrolled in Humana Healthy Horizons in Florida.
